Why behavioral health claims get stuck

Mental health billing has its own habits. Psychotherapy codes such as 90791, 90832, 90834 and 90837 depend on session length and documentation. Many plans carve behavioral health out to a separate administrator, so the card in your client's wallet is not always the payer you bill. Some plans limit visits or need prior authorization, and telehealth sessions need the right place-of-service code and modifier for that payer. Any one of these can turn a session you already delivered into a denial weeks later.
